Responsible for planning, programs, services and information, elder
rights and caregiver services specifically designed to help older adults
and their families. Home and community based program referral including
adult day care for individuals with moderate physical or mental handicaps
who live alone or with families and can attend a day care program. Some
additional referral services include: escort service for errands and
appointments, homemaker assistance, housing assistance, nutrition and
meal programs, personal care, respite services for caregivers, special
